The goal of Home Care is to provide patients with skilled nursing, physical therapy, speech pathology, occupational therapy, and home health aid services, which are provided at the request of a physician or authorized practitioner.
There are two devices utilized by Public Health to make home care easier. The first is Med Partner, which helps patients take their medications correctly. It has 16 places for the patient's pill bottles, which are numbered on the top. The machine is programmed to remind the patient what time to take the medication, which numbered bottle to take and how much to take. The machine does not stop the reminders until the correct pill bottle has been removed and then replaced after the medication has been taken.
The second device is the TeleHealth Monitor, which is installed in a patient's home by his or her nurse. The device is set so that at a particular time every day, the patient's blood pressure, pulse, oxygen levels, and weight measurement is taken. The vitals are then automatically transferred to the office where they are monitored to ensure that the patient's vitals are within their pre-determined parameters. If there is an issue with the patient's vitals, the nurse can then make a visit or the patient's doctor can be notified.

Congestive Heart Failure Guide
As part of a Chronic Disease Management Program, a dedicated committee has put together an informational booklet on Congestive Heart Failure that will be given to patients with CHF. The booklet will be used as a teaching tool to help the patients more aware of their disease. It will also help the patient, families and care-givers to better manage CHF. The overall goal is to reduce acute hospitalizations, the committee will be working on other chronic diseases, such as COPD and Diabetes in the near future.

Description:
Chronic diseases - such as heart disease, cancer, diabetes, stroke, and arthritis - are the leading causes of disability and death in New York State and throughout the United States. More than 40% of New York adults suffer from a chronic disease, and chronic diseases are responsible for 23% of all hospitalizations in New York State. Six out of every 10 deaths in New York State are caused by chronic diseases. Heart disease and cancer account for over half of all deaths in New York State.
Although common and costly, many chronic diseases are also preventable. Many chronic diseases are linked to lifestyle choices that are within your own hands to change. Eating nutritious foods, becoming more physically active and avoiding tobacco can help keep you from developing many of these diseases and conditions. And, even if you already have diabetes, heart disease, arthritis or another chronic condition, eating more healthful food and getting more exercise, whether it's a brisk walk, a bike ride, a jog or a swim, can help you better manage your illness, avoid complications and prolong your life.
Chronic disease prevention is rooted in the modification of risk factors (primary prevention), the detection of chronic diseases in their earliest stages (secondary prevention) and the treatment of chronic disease and attention to disease management and self-management by diagnosed individuals in order to prevent debilitating and costly complications (tertiary prevention).Links:

Lead Poisoning Prevention Program

The aim of the Lead Poisoning Prevention Program is to educate County residents about the hazards associated with lead in an effort to prevent exposures, especially to residents who are most vulnerable to its effects, i.e. children, pregnant women. We also offer lead screenings for children and provide follow up for individuals with elevated lead levels. We also provide information on recalled products that violate the lead paint standard in as timely a manner as possible.

Immunizations
Description:
Many deaths are attributed to vaccine-preventable diseases each year, and they continue to be a threat to the public's health. Vaccines are an effective way to prevent certain diseases and have a low rate of side effects. Franklin County's vaccine programs include:
Vaccines for Children (VFC): This program allows children 0 to 18 years old who are eligible to receive immunizations at a reduced cost.
Adults Hepatitis Vaccination: This program allows adults 18 years of age and older who are eligible to receive Hepatitis A and B immunizations at a reduced cost.
We also provide vaccinations for people aged 18 and older who do not qualify for these programs; there is a charge for these vaccinations.

Rabies
Description:
Franklin County Public Health is responsible for preventing residents from rabies infection. To do this, the County investigates all reported animal to human bites, and animal to animal bites; decides whether an animal should be tested, observed, confined, or quarantined; and determines whether a human potentially exposed to rabies should be treated.
All dog, cat & ferret 3 months of age or older is required to be vaccinated.
Free Rabies Clinics are held throughout the County, with the help of a local veterinarian, throughout the year. Look for our flyers and Facebook page for dates and locations.

Sexually Transmitted Diseases
Description:
Sexually Transmitted Diseases are diseases that are spread from one person to another through anal, vaginal and oral intercourse.
Franklin County Public Health contracts with Planned Parenthood to provide testing of chlamydia, gonnorrea and syphillis, as well as HIV. All test are confidential.
