Isolation and Quarantine Guidelines
Franklin County Public Health recommends that individuals that are in isolation and quarantine follow the guidance published by the New York State Department of Health. The site contains tables for determining the length of an isolation or quarantine, affirmation forms to be provided to employers, and guidance for K-12 schools.
How long do I have to Isolate or Quarantine?
New York State Department of Health has provided a set of tables that you can reference to determine the length of your isolation/quarantine. The tables are published on the above site in the "Guidance and FAQs" section at https://coronavirus.health.ny.gov/new-york-state-contact-tracing#guidance---faqs
Isolation and Quarantine Documents
Franklin County Public Health has provided documents that can be printed and filled out upon receiving a positive test. These forms can be completed as you or your child go through the isolation and quarantine process and shared with parties that may require these documents, such as your employer.
If you are unable to download or print the documents, please call Franklin County Public Heath at 518-481-1709, and choose the option for Isolation and Quarantine (option 2) for assistance.
Health Order to Go into or Remain In Isolation/Quarantine
If you meet the CDC's criteria for isolation and quarantine (Guidance Here) and are required to enter either isolation or quarantine, please fill out the following form:

Affirmation of Isolation Release
Complete this form if you or your child had tested positive for COVID-19 and have been in isolation for the total number of days required by the CDC. (Use a separate form for each positive person.)

Complete this form if you or your child:
- Have been identified as a close contact to a COVID-19 positive person during their contagious period, and
- Was not fully vaccinated at the time of exposure to a COVID-19 positive person during their contagious period, and
- Have been in quarantine for the total number of days required by the CDC.
(Use a separate form for each quarantined person.)
