Isolation Documents

Franklin County Public Health is providing the New York State Department of Health document, below, that can be printed and filled out upon receiving a positive test. This form can be completed as you or your child go through the isolation process and shared with parties that may require these documents, such as your employer.

Health Order/Affirmation to Go Into or Be Released From Isolation

If you meet the CDC's criteria for isolation (https://www.cdc.gov/coronavirus/2019-ncov/your-health/isolation.html) and are required to enter isolation, please fill out the following form:
